Online Authorization Form

In the state of Texas, customers are required to complete an authorization form prior to any repair being done. Here at Best in Show Collision, we have made it easier for you to authorize the estimated repairs online, saving you the trouble of visiting our location.

-ELECTRONIC SIGNATURE & COMMUNICATION AUTHORIZATION- I hereby authorize and consent to electronic transactions and disclosures with Best in Show Collision. I fully agree that my electronic signature shall bear the same force and effect and serve as the legal equivalent of my manual signature for the Best in Show Collision Repair Authorization form. I also authorize communication with Best in Show Collision in respect to my vehicle repair by phone, email or other messaging with an understanding that I have the option to opt out of these communications at my discretion.

-PARTS ORDERING AND INSTALLATION AUTHORIZATION- I authorize Best in Show Collision to procure, order, and install any necessary parts required to complete the approved repairs on my vehicle. This includes the discretion to replace damaged components whether or not visible, with new or quality aftermarket parts as deemed necessary by the collision center’s professional judgment.

-COMMUNICATION AND ADDITIONAL WORK APPROVAL- I acknowledge that unforeseen circumstances may arise during the repair process, and I authorize Best in Show Collision to communicate any additional work or repairs deemed necessary for the comprehensive restoration of my vehicle. I understand that such communication will include a detailed explanation of the additional work and associated costs, and my approval will be sought before proceeding with any supplemental repairs.

-LIABILITY- I acknowledge that Best in Show Collision shall not be held liable for any loss or damage to my vehicle resulting from acts of God, which include but are not limited to natural disasters, severe weather conditions, earthquakes, floods, fires, or other uncontrollable events beyond the reasonable control of the collision center

-PAYMENT TERMS AND RELEASE OF VEHICLE- I understand that payment for the completed repairs and/or services is due upon the release of my vehicle from Best in Show Collision. The total amount due includes the initially estimated cost, as well as any approved additional charges for unforeseen repairs. I acknowledge that failure to settle the invoice in a timely manner may result in additional charges and may impact the release of my vehicle.

I acknowledge that I am providing my electronic signature for the authorization and agreement outlined in this form. I confirm that I have read, understood, and agreed to the terms and conditions specified in this authorization form.
